YPAC Swim Team Medical Release Form
A swimmer must turn in an YPAC Swim Team Medical Release Form before they enter the first practice.  One form must be filled out per swimmer.  These forms are located in a notebook at the front desk.  This notebook will be kept at the YMCA or Ferndale High School – depending on where your child swims, so that coaches will have easy access to this emergency information.  We will also ask for an email address – we encourage you to provide us with an address so that we can get last minute information to you.

 

FINANCIAL RESPONSIBILITY

YMCA MEMBERSHIP
Each swimmer must be a member in good standing of the YMCA. Memberships are on an annual basis and can be purchased at the YMCA office at the beginning of the swim season. The current fee for a Youth General Membership is $120.00 per year, for a Teen General Membership is $240.00 per year and for a Family General membership is $756.00 per year. Membership assistance may be available—contact the YMCA office. Membership can be paid in full or by a monthly EFT. Great news, there is now a four monthly EFT (electronic funds transfer) payment plan for swim team registration as an alternative to full payment.

Swim team fees are also payable at the YMCA office. The fees are based on pool time per swimmer. The current fees for the 2006/2007 season are as follows:
The fees are set by the YMCA and are payable by Monday, October 2nd, 2006.
NEW THIS YEAR YPAC REGISTRATION MAY BE PAID IN 4 EFT PAYMENTS.
In addition to the above fees, a $25.00 per family Concession Fee is also due at registration time. At all home dual meets and invitational our Concession committee will purchase food and drink supplies and will not ask for family food donations.
ADDITIONAL EXPENSES—APPROXIMATE COSTS
All other expenses are paid directly to the swim team (except lodging). Although many of the following items are not mandatory, you can expect to incur some of these costs:

Each child is required to provide his/her own goggles for practices and meets. Having an extra pair in their swim bags is a good idea. Girls must wear swim caps at practice. Boys should wear swim caps if their hair is long and will get in their eyes. At meets, all girls must wear a YPAC team swim cap. If a boy wears a cap at a meet, he must also wear a YPAC team swim cap. This is so that coaches can quickly identify our team swimmers in the pool. A YPAC team cap is given to each child at their first meet.

Depending on which group your swimmer is in, they may be requested (not required) to bring the following equipment to all practices: swim fins, short fins or zoomers, a pull buoy, a kickboard, hand paddles and a mesh bag. All swimmers should bring a water bottle to practice and meets. Questions regarding this equipment should be directed to your age group coach.
